PAYMENT
I accept cash, checks and credit cards in my practice and payment is due at the time of service. I provide the necessary billing information so patients and families can submit out-of-network claims to their insurance company. I recommend patients and families contact their insurance company prior to establishing treatment with me to understand their out-of-network benefits. 


CANCELLATIONS
Please provide at least 24 hours' notice of appointment cancellations or rescheduling.  If appointments are cancelled or rescheduled with less than 24 hours' notice, there will be a $100 late cancellation fee.
