PAYMENT
I accept cash, checks and credit cards in my practice and payment is due at the time of service. I provide the necessary billing information so patients and families can submit out-of-network claims to their insurance company. I recommend patients and families contact their insurance company prior to establishing treatment with me to understand their out-of-network benefits. 


CANCELLATIONS
Please provide at least 24 hours' notice of appointment cancellations or rescheduling.  If appointments are missed, cancelled or rescheduled with less than 24 hours' notice, there will be a fee equal to the amount of the missed appointment.
